- This accredited program will guide pharmacists to empower patients with chronic conditions to effectively self-manage using peer support and reliable on-line resources. This will allow pharmacists to provide quality care to their patients and build trusted relationships. In return, patients will be better equipped to self-manage their chronic disease by leveraging external supports that are impactful and trustworthy.
