Description / Abstract: With the lack of timely access to healthcare, many provinces have expanded the scope of pharmacists to improve the care of people living with chronic conditions, such as type 2 diabetes. By simply applying the guideline recommendations for individuals with type 2 diabetes, pharmacists can potentially change the course of this chronic disease. This program is designed to provide pharmacists with a list of interventions that they can utilize for the management of people with type 2 diabetes and either CVD or CKD.
